APPOINTMENT SCHEDULING Although we try to keep your appointment on schedule, there are occasional emergencies in which the doctor is unavoidably detained. We will make every effort to contact you to reschedule if this should occur, so it is important that we have your phone number(s) on hand. If the doctor is running late at the time of your appointment, you will be informed and given the choice of waiting or rescheduling. We appreciate your patience and understanding in this event. We do appreciate the sensitive nature of many urological conditions. Since our reception area seating is somewhat limited, we ask that you try to limit the number of people who accompany you for your appointment. If you are unable to keep an appointment, please be kind enough to notify us as early as possible at 201.342.6600 so that we may schedule another patient for your time slot. Thank you. Some office procedures (such as cystoscopy, vasectomy, biopsy, ultrasound examinations, etc) require additional time. When scheduling an appointment, please inform our receptionist of the nature of your problem so that the proper amount of time can be allotted for your visit. WHAT TO BRING TO YOUR FIRST APPOINTMENT To save you time in the waiting room, you can print and fill out our patient information form from this web page. To view the form and/or print it out, click here: patient information forms. FINANCES, INSURANCE, AND FEES If you have commercial medical insurance, we will supply you with a receipt for filing your insurance claim. You will be responsible for full payment of your bill if there is a difference between the insurance payment and the amount of the bill. As a courtesy, we will file your hospital claim with your insurance company and you will receive a statement for services rendered. Billing If you are a member of an HMO (health maintenance organization), please provide us with the appropriate forms and your insurance ID card when you register with the receptionist. These forms must be obtained by you from your primary care physician prior to your visit in our office. If you do not have the necessary referral forms from your primary care doctor, your office visit will need to be rescheduled until such time as you have these available. Your co-payment is due at the time of service. We will be happy to work with you in establishing a payment program that will not create a hardship in the event of a larger fee not covered by insurance. Our fees for office visits, procedures, and surgery are on file in the office. If you have a specific question about our charges, please feel free to ask us. |
